WSACS Intra-Abdominal Hypertension (IAH Grades I–IV) & Abdominal Perfusion Pressure Formula

Calculate Abdominal Perfusion Pressure APP = MAP - IAP (target >= 60 mmHg) and classify trans-vesical bladder pressure into WSACS Intra-Abdominal Hypertension Grades I–IV and Abdominal Compartment Syndrome (ACS > 20 mmHg + new organ failure).

Just as Cerebral Perfusion Pressure equals MAP - ICP, visceral organ perfusion depends on APP = MAP - IAP!

Standardized by the World Society of the Abdominal Compartment Syndrome (WSACS — Kirkpatrick et al., 2013, Intensive Care Medicine), Intra-Abdominal Pressure (IAP) is measured via end-expiratory supine trans-vesical (Foley bladder) pressure zeroed at the iliac crest mid-axillary line (normal ICU baseline 5–7 mmHg). Sustained IAP >= 12 mmHg defines Intra-Abdominal Hypertension (IAH) across four grades: Grade I (12–15 mmHg), Grade II (16–20 mmHg), Grade III (21–25 mmHg), and Grade IV (> 25 mmHg), while sustained IAP > 20 mmHg WITH new organ dysfunction/failure defines Abdominal Compartment Syndrome (ACS)!

Analogous to cerebral perfusion pressure (CPP = MAP - ICP), Michael L. Cheatham et al. (2000, Journal of Trauma) demonstrated that Abdominal Perfusion Pressure APP = MAP - IAP predicts splanchnic/renal perfusion and survival better than MAP or IAP alone (resuscitation target APP >= 60 mmHg)! At MAP = 68 mmHg and IAP = 22 mmHg, APP = 46 mmHg (< 60 mmHg renal/gut ischemia!) with Grade III IAH!

Abdominal Perfusion Pressure APP (mmHg) = MAP - IAP

Subtracts end-expiratory bladder pressure IAP (B3, mmHg) from Mean Arterial Pressure MAP (B2, mmHg).

WSACS 2013 Intra-Abdominal Hypertension Grade Classifier (Normal, Grades I–IV)

Classifies supine trans-vesical IAP into Normal (< 12), Grade I (12–15), Grade II (16–20), Grade III (21–25), or Grade IV (> 25 mmHg).
