Winter-Tozer Albumin-Corrected Phenytoin & ESRD Formula

Calculate Protein-Binding Normalization Factor, Adjusted Normal-Albumin Phenytoin Concentration (mg/L), Estimated Free Unbound Phenytoin (mg/L), and Therapeutic Range Alert (10–20 mg/L).

Why a "low" 7.5 mg/L total phenytoin lab is already THERAPEUTIC at Albumin 2.2—and TOXIC in dialysis ESRD!

Phenytoin is ~90% bound to serum albumin in healthy adults (normal albumin ~4.4 g/dL, where standard hospital immunoassays report Total Phenytoin target 10.0–20.0 mg/L corresponding to Free Unbound Phenytoin 1.0–2.0 mg/L). In critically ill, cirrhotic, or malnourished patients with hypoalbuminemia, unbound pharmacologically active fraction surges—so blindly escalating dose on a "low" total level triggers saturable Michaelis-Menten neurotoxicity (nystagmus, ataxia, coma)! The Sheiner-Tozer / Winter-Tozer Equation normalizes Measured Total Phenytoin C_total (mg/L) to an equivalent normal-albumin level: C_corr = C_total / (0.2 * Albumin_g_dL + 0.1) when renal function is preserved (CrCl >= 10 mL/min), switching the albumin binding slope from 0.2 down to 0.1 in uremic ESRD / dialysis (CrCl < 10 mL/min, where uremic toxins displace phenytoin from albumin!): C_corr,ESRD = C_total / (0.1 * Albumin_g_dL + 0.1)!

Verify on a Neuro-ICU Seizure Patient—Measured_Total_Phenytoin = 7.50 mg/L (B2, misleadingly < 10 mg/L!), Serum_Albumin = 2.20 g/dL (B3), and Uremic_ESRD_Flag = 0 (preserved renal function, B4): Binding Denominator equals 0.2 * 2.20 + 0.1 = 0.54 (E2), Corrected Total Phenytoin equals 7.50 / 0.54 = 13.89 mg/L (E3), Estimated Free Phenytoin is 1.39 mg/L (E4), and Clinical Status reads "THERAPEUTIC (10-20)" (E5—NO dose increase needed)!

Winter-Tozer Normal-Renal vs Uremic-ESRD Corrected Phenytoin Formula

Validate Measured_Total > 0, Serum_Albumin in [0.5, 6.0] g/dL, and ESRD_Flag in {0, 1} (always order a direct ultrafiltration Free Phenytoin assay when co-administered with valproic acid displacement)!

=LET(cp_t, B2, al_s, B3, es_r, B4, IF(OR(cp_t <= 0, al_s < 0.5, al_s > 6, AND(es_r <> 0, es_r <> 1)), "Invalid TDM Input", LET(cf_b, IF(es_r = 1, 0.1, 0.2), cc_r, cp_t / (cf_b * al_s + 0.1), IF(cc_r < 10, "SUBTHERAPEUTIC (<10)", IF(cc_r <= 20, "THERAPEUTIC (10-20)", "SUPRATHERAPEUTIC TOXIC (>20)")))))

Uremic ESRD Binding-Displacement Toxicity Flip Check (Set ESRD_Flag = 1)

Toggle Uremic_ESRD_Flag B4 from 0 to 1 (CrCl < 10 mL/min hemodialysis patient with the exact same Total Phenytoin 7.50 mg/L and Albumin 2.20 g/dL): Binding Denominator E2 drops from 0.54 to 0.32, Corrected Phenytoin E3 jumps to 23.44 mg/L (Estimated Free = 2.34 mg/L in E4), flipping E5 immediately to "SUPRATHERAPEUTIC TOXIC (>20)"!

=IF(OR(B2 <= 0, B3 < 0.5, B3 > 6, AND(B4 <> 0, B4 <> 1)), "Invalid TDM Input", ROUND(B2 / (IF(B4 = 1, 0.1, 0.2) * B3 + 0.1), 2))
