Vienna CATS Score for Cancer-Associated Venous Thromboembolism (0–8)
Augment the Khorana chemotherapy VTE score (0–6) with D-dimer (>= 1.44 µg/mL, +1) and soluble P-selectin (>= 53.1 ng/mL, +1) to stratify 6-month cancer VTE incidence.
Biomarker-enhanced Khorana scoring—sharpening 6-month oncology VTE prediction from 1.0% to 35.0%!
While the clinical Khorana score (0–6 points across tumor site, platelets >= 350, Hb < 10 / ESA, WBC > 11, BMI >= 35) identifies ambulatory cancer VTE risk, Cihan Ay et al. (Blood, 2010 — Vienna Cancer and Thrombosis Study, CATS) demonstrated that adding two coagulation biomarkers—elevated D-dimer >= 1.44 µg/mL (+1 point) and soluble P-selectin >= 53.1 ng/mL (+1 point)—expands the scale to 0–8 points and sharply separates 6-month symptomatic VTE risk from 1.0% (score 0) up to 35.0% (score >= 5)!
Evaluate a pancreatic cancer patient starting chemotherapy with Khorana base score 2, D-dimer 1.85 µg/mL (+1), and sP-selectin 61.4 ng/mL (+1): Vienna CATS = 4 points (High VTE Risk, ~10%–20% 6-month VTE incidence; rising to >= 5 Very High Risk ~35% if Khorana base reaches 3)!
| Khorana Base (0–6) | D-dimer (>= 1.44 µg/mL) | sP-selectin (>= 53.1 ng/mL) | Vienna CATS Score & 6-Month VTE Tier |
|---|---|---|---|
| 0 pts | 0.65 µg/mL (+0) | 38.0 ng/mL (+0) | 0 pts — Low-to-Intermediate VTE Risk (<5%) |
| 2 pts | 1.85 µg/mL (+1) | 61.4 ng/mL (+1) | 4 pts — High VTE Risk (10%–20% — Consider Thromboprophylaxis) |
| 3 pts | 1.85 µg/mL (+1) | 61.4 ng/mL (+1) | 5 pts — Very High VTE Risk (~35% at 6 Months) |
Vienna CATS Total Biomarker-Enhanced VTE Score (0–8 Points)
Adds +1 for D-dimer B3 >= 1.44 µg/mL and +1 for soluble P-selectin B4 >= 53.1 ng/mL to Khorana clinical base score B2 (0–6).
=IF(OR(B2 < 0, B2 > 6, B3 < 0, B4 < 0), "Invalid Vienna CATS Inputs", B2 + IF(B3 >= 1.44, 1, 0) + IF(B4 >= 53.1, 1, 0))6-Month Ambulatory Oncology VTE Risk Classification
Stratifies Vienna CATS totals into Low-to-Intermediate (0–2), High (3–4), and Very High (>= 5, ~35% 6-month VTE) tiers.
