Talcott Risk Classification for Febrile Neutropenia (Groups I–IV Triage)

Classify chemotherapy-induced febrile neutropenia into Talcott Groups I–IV to identify Group IV low-risk outpatients (< 5% serious medical complication rate).

Hierarchical sequential exclusion—why outpatient febrile neutropenia requires clearing all three Talcott gates!

Before MASCC and CISNE existed, James A. Talcott et al. (Archives of Internal Medicine, 1988; prospectively validated in Journal of Clinical Oncology, 1992) established the foundational hierarchical rule separating high-risk from outpatient-eligible febrile neutropenia! Patients are assigned sequentially to the highest-priority group met: Group I (already inpatient when fever and neutropenia begin — ~34% serious complication rate), Group II (outpatient onset WITH serious acute comorbidity requiring hospitalization such as hypotension, respiratory failure, or altered mental status — ~55% complications), Group III (outpatient without comorbidity but WITH uncontrolled/progressive cancer or induction leukemia — ~31% complications), and Group IV (outpatient onset, no acute comorbidity, controlled malignancy — <= 2%–5% serious complications, outpatient oral/ambulatory antibiotic candidate)!

Because Talcott’s groups are mutually exclusive hierarchical gates rather than additive points, nested IF logic mirrors oncology triage protocols cleanly without false reassurance!

Talcott Hierarchical Febrile Neutropenia Group (I–IV)

Evaluates inpatient status B2, serious acute comorbidity B3, and uncontrolled malignancy B4 in strict Talcott I -> II -> III -> IV priority order.

=IF(OR(B2 < 0, B2 > 1, B3 < 0, B3 > 1, B4 < 0, B4 > 1), "Binary Flags Must Be 0 or 1", IF(B2 = 1, "Group I (Inpatient Onset — High Risk)", IF(B3 = 1, "Group II (Acute Comorbidity — High Risk)", IF(B4 = 1, "Group III (Uncontrolled Cancer — High Risk)", "Group IV (Low Risk — Outpatient Eligible)"))))

Count of High-Risk Exclusion Criteria Present (0–3)

Sums B2:B4 (must equal 0 for Group IV outpatient eligibility).
