Spetzler-Martin Brain AVM Grading Scale (Grades I–V & Spetzler-Ponce Class)
Calculate Robert Spetzler and Neil Martin’s cerebral arteriovenous malformation surgical risk score (1–5: Nidus Size <3 cm = 1, 3–6 cm = 2, >6 cm = 3; Eloquent Cortex = +1; Deep Venous Drainage = +1) and 3-tier Spetzler-Ponce treatment class.
How three angiographic features predict microsurgical AVM resection morbidity!
Robert F. Spetzler and Neil A. Martin (Journal of Neurosurgery, 1986) revolutionized cerebrovascular surgery by proving that operative morbidity of resecting a brain arteriovenous malformation (AVM) is governed by just three anatomical features summing from Grade I (1 pt) to Grade V (5 pts): (1) Largest Nidus Diameter on angiography (< 3.0 cm = 1 pt, 3.0–6.0 cm = 2 pts, > 6.0 cm = 3 pts), (2) Eloquence of Adjacent Brain—sensorimotor, language, visual cortex, thalamus/hypothalamus, internal capsule, brainstem, cerebellar peduncles, or deep cerebellar nuclei (Non-Eloquent = 0, Eloquent = 1 pt), and (3) Pattern of Venous Drainage (Superficial cortical veins only = 0, Any Deep drainage into internal cerebral/basal/precentral cerebellar veins = 1 pt)!
In 2011, Spetzler and Ponce consolidated the five grades into three actionable management classes: Class A (Grades I & II: 1–2 pts -> microsurgical resection favored), Class B (Grade III: 3 pts -> multimodal embolization + surgery or stereotactic radiosurgery), and Class C (Grades IV & V: 4–5 pts -> conservative observation generally favored unless recurrent hemorrhage)!
| Spetzler-Martin Sum (1–5) | Spetzler-Ponce 3-Tier Class | Standard Cerebrovascular Strategy |
|---|---|---|
| 1 – 2 Points (Grade I – II) | Class A (Low Operative Risk) | Microsurgical Resection First-Line |
| 3 Points (Grade III; e.g., 3.5 cm + Eloquent) | Class B (Intermediate Risk) | Multimodal Embolization / Radiosurgery / Surgery |
| 4 – 5 Points (Grade IV – V) | Class C (High Surgical Morbidity) | Conservative Management (Unless Bleed Rescue) |
Spetzler-Martin Brain AVM Numeric Grade (1 to 5)
Converts nidus diameter B2 (cm) into 1 (<3), 2 (3–6), or 3 (>6) points and adds binary Eloquence B3 and Deep Venous Drainage B4.
=IF(OR(B2 <= 0, NOT(OR(B3 = 0, B3 = 1)), NOT(OR(B4 = 0, B4 = 1))), "Invalid AVM Inputs", IF(B2 < 3, 1, IF(B2 <= 6, 2, 3)) + B3 + B4)Spetzler-Ponce 3-Tier Management Class (Class A, B, or C)
Maps total score 1–2 -> Class A, 3 -> Class B, and 4–5 -> Class C.
=IF(OR(B2 <= 0, NOT(OR(B3 = 0, B3 = 1)), NOT(OR(B4 = 0, B4 = 1))), "Invalid AVM Inputs", IF(IF(B2 < 3, 1, IF(B2 <= 6, 2, 3)) + B3 + B4 <= 2, "Spetzler-Ponce Class A (Grades I-II: Microsurgery Favored)", IF(IF(B2 < 3, 1, IF(B2 <= 6, 2, 3)) + B3 + B4 = 3, "Spetzler-Ponce Class B (Grade III: Multimodal Therapy)", "Spetzler-Ponce Class C (Grades IV-V: High Surgical Risk)")))Try the example
