PRAM Score Formula: Pediatric Respiratory Assessment Measure for Acute Asthma (0 to 12)

Grade acute pediatric asthma exacerbation severity across toddlers and teenagers (ages 1–17) using Ducharme & Chalut’s 5-sign PRAM score (0–12 points) without spirometry.

Why Canadian Pediatric Society asthma pathways standardize on the 5-item, 12-point PRAM score!

Preschoolers and acutely dyspneic children rarely perform reliable FEV1 spirometry in the emergency department! Danielle Chalut, Francine M. Ducharme, et al. (2000, Journal of Pediatrics; validated ages 1–17 in 2008) developed the 0–12 Pediatric Respiratory Assessment Measure (PRAM) from 5 reproducible bedside signs: Suprasternal retractions (0 or 2 pts), Scalene muscle contraction (0 or 2 pts), Air entry at lung bases (Normal: 0, Decreased at bases: 1, Widespread decrease: 2, Absent/minimal: 3), Wheezing (Absent: 0, Expiratory only: 1, Inspiratory + expiratory: 2, Audible without stethoscope OR silent chest: 3), and Room-air O2 saturation (>= 95%: 0, 92–94%: 1, < 92%: 2)!

PRAM 0–3 defines Mild asthma (inhaled SABA alone), PRAM 4–7 Moderate asthma (SABA + systemic dexamethasone/prednisolone), and PRAM 8–12 Severe impending respiratory failure (back-to-back SABA + ipratropium + early systemic steroid + IV magnesium sulfate consideration)!

PRAM Total Score (0–12)Asthma Exacerbation SeverityPediatric Emergency Bronchodilator & Steroid Protocol
PRAM 0–3 (Mild)Mild BronchospasmSalbutamol MDI + Spacer; Reassess for Discharge
PRAM 4–7 (Moderate)Moderate Airway ObstructionSalbutamol × 3 + Oral Dexamethasone / Prednisolone
PRAM 8–12 (Severe, e.g., 9)Severe / Impending FailureContinuous/3× SABA + Ipratropium + Steroid + IV MgSO4

Total 5-Sign PRAM Pediatric Asthma Score (0 to 12 Points)

Sums Suprasternal (B2, 0 or 2), Scalene (B3, 0 or 2), Air Entry (B4, 0–3), Wheezing (B5, 0–3), and O2 Saturation (B6, 0–2) points.

=IF(OR(MIN(B2:B6) < 0, NOT(OR(B2 = 0, B2 = 2)), NOT(OR(B3 = 0, B3 = 2)), B4 > 3, B5 > 3, B6 > 2), "Invalid PRAM Sign Score", SUM(B2:B6))

PRAM Acute Asthma Severity Tier (Mild 0–3, Moderate 4–7, Severe 8–12)

Classifies bedside PRAM total into Mild, Moderate, or Severe pediatric asthma pathway.

=IF(OR(MIN(B2:B6) < 0, NOT(OR(B2 = 0, B2 = 2)), NOT(OR(B3 = 0, B3 = 2)), B4 > 3, B5 > 3, B6 > 2), "Invalid PRAM Sign Score", IF(SUM(B2:B6) >= 8, "Severe Asthma (PRAM 8–12 — SABA + Ipratropium + Steroid + IV MgSO4)", IF(SUM(B2:B6) >= 4, "Moderate Asthma (PRAM 4–7 — SABA + Systemic Corticosteroid)", "Mild Asthma (PRAM 0–3 — Inhaled SABA)")))
