PELOD-2 (Pediatric Logistic Organ Dysfunction-2) Score & Mortality
Sum 5 pediatric organ-system dysfunction subscores (Neurologic, Cardiovascular, Renal, Respiratory, Hematologic; 0–33) and compute Leteurtre et al.’s (2013) PICU mortality probability = 100 / (1 + EXP(6.61 - 0.47 * PELOD-2)).
Continuous integer organ scoring—replacing PELOD-1’s bimodal gaps in pediatric sepsis and MODS!
The original 1999 PELOD score suffered from discontinuous jumps (skipping from 1 to 10 to 20 points). Stéphane Leteurtre et al. (Critical Care Medicine, 2013; n = 3,671 PICU patients) recalibrated PELOD-2 onto a smooth 0–33 integer scale across 5 organ systems—Neurologic (GCS & pupillary reaction, 0–10), Cardiovascular (lactate & age-adjusted MAP, 0–10), Renal (age-adjusted creatinine, 0–3), Respiratory (PaO2/FiO2, PaCO2, & invasive ventilation, 0–6), and Hematologic (WBC & platelets, 0–4)—converting total PELOD-2 score S into PICU mortality probability P(%) = 100 / (1 + EXP(6.61 - 0.47 * S))!
Track a septic shock child with Neurologic = 2, Cardiovascular = 3, Renal = 2, Respiratory = 2, and Hematologic = 2: PELOD-2 = 11 points yields 19.15% predicted PICU mortality—dropping dramatically to 0.87% as organ recovery lowers total PELOD-2 to 4!
| 5-Organ Subscore Vector [Neuro, CV, Renal, Resp, Heme] | PELOD-2 Score (0–33) | Log-Odds (-6.61 + 0.47*S) | Predicted PICU Mortality (%) |
|---|---|---|---|
| [1, 1, 1, 1, 0] — Mild Resolving MODS | 4 pts | -4.73 | 0.87% |

PELOD-2 Predicted PICU Mortality Probability (%)
Evaluates Leteurtre’s 2013 logistic equation 100 / (1 + EXP(6.61 - 0.47 * SUM(B2:B6))) across the 5 organ dysfunction subscores B2:B6.

Sums Neurologic (B2), Cardiovascular (B3), Renal (B4), Respiratory (B5), and Hematologic (B6) subscores.
