Originally introduced by J. Antonio Aldrete and Denton Kroulik in 1970 (modeled on Virginia Apgar’s newborn 0–10 score) and updated in 1995 to replace subjective skin-color inspection with objective pulse oximetry (SpO2), the Modified Aldrete Score grades five physiological recovery domains from 0 to 2 points (0–10 total): (1) Motor Activity (moves 4 extremities = 2, 2 extremities = 1, 0 = 0), (2) Respiration (deep breath & cough = 2, dyspnea/limited = 1, apnea = 0), (3) Circulation (BP within +/- 20% of pre-anesthetic = 2, +/- 20–49% = 1, >= 50% shift = 0), (4) Consciousness (fully awake = 2, arousable on calling = 1, unresponsive = 0), and (5) Oxygenation (SpO2 > 92% on room air = 2, > 90% with O2 = 1, < 90% despite O2 = 0)!
Standard Phase-I PACU transfer to a surgical ward requires BOTH a total Modified Aldrete Score >= 9 AND zero 0-point critical subscores (MIN > 0)—so an apneic, severely hypoxic (SpO2 < 90% on O2), or completely unresponsive patient can never be discharged prematurely!
