Mentzer Index (MCV / RBC) & Green-King Index: Beta-Thalassemia Trait vs. Iron Deficiency

Differentiate microcytic beta-thalassemia minor (< 13 Mentzer, < 65 Green-King) from iron-deficiency anemia (> 13 Mentzer, >= 65 Green-King) directly from routine CBC indices.

Why beta-thalassemia minor produces compensatory erythrocytosis (high RBC -> Mentzer < 13) while iron deficiency starves marrow RBC output!

Both iron-deficiency anemia (IDA) and beta-thalassemia trait present with low MCV (< 80 fL)—and mistakenly prescribing empiric iron to a thalassemia carrier risks iron overload without fixing microcytosis! William C. Mentzer Jr. (1973, The Lancet) observed that thalassemia trait preserves erythropoietin-driven marrow proliferation (RBC often >= 5.5 × 10^6/μL despite MCV 60–68 fL), driving Mentzer Index = MCV / RBC < 13, whereas iron-deficient marrow drops RBC count proportionally so MCV / RBC > 13!

Pairing Mentzer with the RDW-incorporating Green & King Index = (MCV^2 * RDW) / (100 * Hb) (< 65 favors uniform microcytic beta-thalassemia trait; >= 65 favors anisocytotic IDA with high RDW) delivers top meta-analytic discrimination before hemoglobin electrophoresis! For MCV = 64 fL, RBC = 5.80 × 10^6/μL, Hb = 11.4 g/dL, and RDW = 13.2%, Mentzer = 11.03 (< 13) and Green-King = 47.43 (< 65)—flipping to Mentzer = 17.44 (> 13, IDA) when MCV = 68 fL and RBC = 3.90!

Mentzer Microcytic Discrimination Index = MCV (fL) / RBC (10^6/μL)

Computes MCV / RBC from B2 and B3 across microcytic CBCs (MCV < 80 fL; < 13 suggests beta-thalassemia trait, > 13 suggests IDA).

=IF(OR(B2 <= 0, B2 >= 80, B3 <= 0, B4 <= 0, B5 <= 0), "Requires Microcytosis (MCV < 80 fL)", ROUND(B2 / B3, 2))

Green & King Red-Cell Distribution Index = (MCV² * RDW) / (100 * Hb)

Combines MCV (B2), RDW % (B5), and Hemoglobin g/dL (B4); values < 65 favor beta-thalassemia trait over IDA.

=IF(OR(B2 <= 0, B2 >= 80, B3 <= 0, B4 <= 0, B5 <= 0), "Requires Microcytosis (MCV < 80 fL)", ROUND(((B2 ^ 2) * B5) / (100 * B4), 2))
