Distilled from the 15-item IIEF by Raymond C. Rosen et al. (International Journal of Impotence Research, 1999), the 5-item IIEF-5—also known as the Sexual Health Inventory for Men (SHIM)—rates five six-month functional domains from 1 to 5 (total 5–25 points among sexually active men): (Q1) Confidence in getting and keeping an erection, (Q2) Erections firm enough for penetration, (Q3) Maintenance of erection after penetration, (Q4) Difficulty maintaining erection to completion, and (Q5) Intercourse satisfaction!
Using the 21-point diagnostic cutoff (sensitivity 98%, specificity 88%), AUA and EAU urology trials classify total IIEF-5 into five clinical strata: 22–25 = No Erectile Dysfunction, 17–21 = Mild ED, 12–16 = Mild-to-Moderate ED, 8–11 = Moderate ED, and 5–7 = Severe ED!
