ICIQ-UI Short Form Urinary Incontinence Severity Score (0–21)

Score the International Consultation on Incontinence Questionnaire Short Form (ICIQ-UI SF, 0–21) across frequency (0–5), even-weighted volume (0/2/4/6), and QoL impact (0–10).

Catching even-weighted leakage volume (0, 2, 4, 6) and Klovning’s validated 0–21 severity bands!

Developed by Kerry Avery et al. (Neurourology and Urodynamics, 2004) and stratified into clinical severity bands by Atle Klovning et al. (Urology, 2009), the ICIQ-UI Short Form is the Grade-A international patient-reported outcome measure for stress, urgency, and mixed urinary incontinence! Its 0–21 sum combines Q3 Leakage Frequency (0–5), Q4 Perceived Leakage Amount scored STRICTLY as even weights (0 = none, 2 = small, 4 = moderate, 6 = large), and Q5 Everyday-Life Interference VAS (0–10) — whereas Q6 self-reported triggers remain unscored qualitative subtyping!

Score a patient leaking once daily (Q3 = 3), a moderate amount (Q4 = 4), with 7/10 daily-life interference (Q5 = 7): ICIQ-UI SF = 14 / 21 (Severe Urinary Incontinence, 13–18), with MOD(Q4, 2) <> 0 catching accidental 1–3 ordinal miscoding of Q4 immediately!

ICIQ-UI Short Form Total Score (0–21) with Even-Volume Guard

Sums Frequency B2 (0–5), even-weighted Volume B3 (0, 2, 4, or 6 enforced via MOD(B3, 2) = 0), and Interference VAS B4 (0–10).

=IF(OR(B2 < 0, B2 > 5, MOD(B3, 2) <> 0, B3 < 0, B3 > 6, B4 < 0, B4 > 10), "Invalid ICIQ-UI-SF Item Score", B2 + B3 + B4)

Klovning Validated ICIQ-UI SF Severity Tier

Classifies valid 0–21 scores into Continence (0), Slight (1–5), Moderate (6–12), Severe (13–18), and Very Severe (19–21).
