HOMA-IR & QUICKI Fasting Insulin Resistance Indices Formula

Calculate Homeostatic Model Assessment of Insulin Resistance HOMA-IR = (Insulin_uIUmL * Glucose_mgdL) / 405 (or / 22.5 in mmol/L) and Quantitative Insulin Sensitivity Check Index QUICKI = 1 / (LOG10(Insulin) + LOG10(Glucose_mgdL)).

Quantify metabolic insulin resistance and sensitivity from a single fasting draw!

The gold-standard hyperinsulinemic-euglycemic clamp is invasive and research-intensive—so endocrinologists and metabolic trials quantify insulin resistance from a single morning fasting blood sample using Matthews's Homeostatic Model Assessment: HOMA-IR = (Fasting_Insulin_uIUmL * Fasting_Glucose_mgdL) / 405 (when fasting glucose is reported in SI mmol/L, divide by 22.5 instead because 405 / 18 = 22.5)!

Because raw insulin distributions are right-skewed, Katz's Quantitative Insulin Sensitivity Check Index applies a base-10 log transform that correlates linearly with glucose clamp sensitivity: QUICKI = 1 / (LOG10(Fasting_Insulin_uIUmL) + LOG10(Fasting_Glucose_mgdL))! Healthy insulin-sensitive adults typically exhibit HOMA-IR < 1.5–2.0 and QUICKI > 0.35, whereas HOMA-IR >= 2.5–3.0 and QUICKI < 0.33 indicate clinical insulin resistance!

HOMA-IR Score from Fasting Insulin (uIU/mL) & Glucose (mg/dL)

Evaluates (Insulin * Glucose_mgdL) / 405 after checking positive fasting lab inputs.

=IF(OR(B2 <= 0, B3 <= 0), "Invalid Fasting Lab Input", ROUND((B2 * B3) / 405, 2))

QUICKI Quantitative Insulin Sensitivity Check Index

Computes 1 / (LOG10(Insulin) + LOG10(Glucose_mgdL)), guarding against LOG10 sum <= 0.

=IF(OR(B2 <= 0, B3 <= 0, (LOG10(B2) + LOG10(B3)) <= 0), "Invalid Fasting Lab Input", ROUND(1 / (LOG10(B2) + LOG10(B3)), 4))
