CRUSADE In-Hospital Major Bleeding Score for NSTEMI (1–96)

Calculate Sumeet Subherwal’s 8-domain CRUSADE Bleeding Risk Score (1–96 points across Hematocrit, CrCl, Heart Rate, Sex, CHF Signs, Prior Vascular Disease, Diabetes, and Systolic BP) to tailor radial access and antithrombotic dosing in NSTEMI.

Balance ischemic plaque protection against major NSTEMI hemorrhage!

Aggressive dual antiplatelet therapy, heparin/bivalirudin, and invasive angiography slash NSTEMI myocardial infarction—yet in-hospital major bleeding carries the SAME mortality hazard as a recurrent heart attack! Sumeet Subherwal et al. (Circulation 2009, n = 71,277 CRUSADE registry NSTEMI patients) developed the 1–96 point CRUSADE Bleeding Score across four continuous nomogram subscores—Baseline Hematocrit (0–9 pts), Cockcroft-Gault Creatinine Clearance (1–39 pts, heaviest single predictor!), Heart Rate (0–11 pts), and Systolic BP (1–10 pts)—plus four binary risk flags: Female Sex (+8), Signs of CHF at presentation (+7), Prior Vascular Disease PAD/stroke (+6), and Diabetes Mellitus (+6)!

Validated CRUSADE quintiles stratify in-hospital major bleeding sharply: Very Low (<= 20 pts: 3.1%), Low (21–30 pts: 5.5%), Moderate (31–40 pts: 8.6%), High (41–50 pts: 11.9%), and Very High (> 50 pts: 19.5%—mandating transradial catheterization, PPI gastroprotection, renal-adjusted anticoagulation, and avoidance of routine GP IIb/IIIa inhibitors)!

Total 8-Variable CRUSADE Bleeding Risk Score (1–96 Points)

Combines the four continuous nomogram subscores (Hct 0–9, CrCl 1–39, HR 0–11, SBP 1–10) with weighted binary flags Female (+8), CHF (+7), Prior Vascular (+6), and Diabetes (+6).
