2021 CKD-EPI Creatinine eGFR and GFR Categories in Excel
Practice the adult 2021 race-free creatinine equation, validate units and inputs, and classify the unrounded estimate without treating it as a CKD diagnosis.
Inputs and equation
Use IDMS-standardized serum creatinine in mg/dL in B2, adult age in years in B3, and the equation sex parameter F or M in B4. Convert creatinine from micromol/L to mg/dL by dividing by 88.4 before entry. This lesson implements the 2021 creatinine-only CKD-EPI equation, not MDRD.
The equation uses kappa 0.7 and alpha -0.241 for F, or kappa 0.9 and alpha -0.302 for M, with the female multiplier 1.012. Output units are mL/min/1.73 m². B6 keeps the unrounded result. Invalid sex labels are rejected instead of silently selecting the male branch.
=IF(OR(COUNT(B2:B3)<>2,B2<=0,B3<18,AND(B4<>"F",B4<>"M")),"Use numeric Scr > 0, adult age >= 18 and sex F or M",LET(kap,IF(B4="F",0.7,0.9),alp,IF(B4="F",-0.241,-0.302),sexf,IF(B4="F",1.012,1),142*MIN(B2/kap,1)^alp*MAX(B2/kap,1)^(-1.2)*0.9938^B3*sexf))GFR category, not a standalone diagnosis
B7 classifies the unrounded B6: G1 >=90, G2 60 to <90, G3a 45 to <60, G3b 30 to <45, G4 15 to <30, G5 <15. Round only for display; rounding 59.96 to 60 must not change the underlying category.
CKD requires persistent abnormalities over at least three months and clinical assessment. G1 or G2 alone, without evidence of kidney damage, is not CKD. A single low estimate does not establish chronic disease. Cause and albuminuria also matter.
=IF(ISNUMBER(B6),IFS(B6>=90,"G1",B6>=60,"G2",B6>=45,"G3a",B6>=30,"G3b",B6>=15,"G4",TRUE,"G5"),B6)Try the example safely
The synthetic 55-year-old F example with creatinine 0.8 produces about 87.0 (G2). Change all three inputs to 1.8, 65, M for about 41.3 (G3b). These labels describe numerical categories, not diagnoses of the example subjects.
This is an educational spreadsheet, not a diagnostic or medication-dosing tool. Creatinine-based estimates have limitations, especially when creatinine is changing rapidly or muscle mass differs substantially. Clinical decisions need professional interpretation; NIDDK also describes combined creatinine-cystatin C estimation. Do not enter identifiable patient information in a shared practice sheet.
