Age-Adjusted Charlson Comorbidity Index (CCI) & 10-Year Survival Formula

Calculate the Age-Adjusted Charlson Comorbidity Index (adding +1 point per decade from age 50 up to +4 at age >= 80, plus 1-, 2-, 3-, and 6-point comorbidity weights) and estimated 10-year survival 100 * (0.983 ^ EXP(CCI * 0.9)).

Quantify longitudinal comorbidity burden in clinical trials, surgery, and oncology!

Mary Charlson et al. (1987, 1994) developed the most widely cited comorbidity risk-adjustment score in clinical epidemiology—assigning weighted points across 17 chronic conditions based on adjusted relative mortality hazard: 1 point (MI, CHF, PVD, CVA/TIA, dementia, COPD, connective tissue disease, peptic ulcer, mild liver disease, uncomplicated diabetes), 2 points (hemiplegia, moderate-to-severe CKD, diabetes with end-organ damage, localized solid tumor, leukemia, lymphoma), 3 points (moderate-to-severe liver disease), and 6 points (metastatic solid tumor or AIDS), plus +1 age point per decade starting at age 50 (50–59 = +1, 60–69 = +2, 70–79 = +3, >= 80 = +4)!

Charlson's proportional-hazards survival equation converts total Age-Adjusted CCI into estimated 10-year survival probability: S_10yr(%) = 100 * (0.983 ^ EXP(CCI * 0.9))! Watch the double-exponential cliff: CCI = 0 predicts 98.30% 10-year survival, CCI = 2 (e.g., age 55 [+1] plus CHF [+1]) predicts 90.15%, CCI = 4 drops to 53.39%, and CCI = 6 plunges to 2.25%!

Age-Adjusted Charlson Comorbidity Index (CCI Total Points)

Computes age decade points MIN(4, MAX(0, INT((Age - 40) / 10))) plus weighted 1-, 2-, 3-, and 6-point condition counts.

=IF(OR(B2 < 18, MIN(B3:B6) < 0), "Invalid Patient Inputs", MIN(4, MAX(0, INT((B2 - 40) / 10))) + B3 + (2 * B4) + (3 * B5) + (6 * B6))

Charlson Estimated 10-Year Survival Percentage: 100 * (0.983 ^ EXP(CCI * 0.9))

Evaluates Charlson's Cox proportional-hazards 10-year survival curve from the total CCI score.

=IF(OR(B2 < 18, MIN(B3:B6) < 0), "Invalid Patient Inputs", ROUND(100 * (0.983 ^ EXP((MIN(4, MAX(0, INT((B2 - 40) / 10))) + B3 + (2 * B4) + (3 * B5) + (6 * B6)) * 0.9)), 2))
