Canadian C-Spine Rule (CCR): 3-Step Cervical Spine Imaging Decision Formula
Evaluate Ian Stiell's 3-step Canadian C-Spine Rule (High-Risk Factor -> Low-Risk Factor Allowing Assessment -> Active 45-Degree Left/Right Neck Rotation) for alert, stable blunt trauma patients (GCS = 15).
Achieve 99.4% sensitivity with 3.5x higher specificity than NEXUS!
In alert (GCS = 15), hemodynamically stable adult blunt trauma patients, ordering routine cervical spine CT scans on everyone with minor neck soreness exposes thousands of low-risk patients to thyroid radiation! Ian Stiell's landmark 8,924-patient Canadian C-Spine Rule (JAMA 2001) achieved 100% sensitivity in derivation and substantially higher specificity (42.5% vs 12.9% in head-to-head NEJM comparison!) than NEXUS via a strict 3-step cascade!
Step 1 checks ANY High-Risk Factor (Age >= 65, dangerous mechanism, or extremity paresthesias): if present (= 1), mandate C-spine imaging immediately without testing range of motion! Step 2 checks whether AT LEAST ONE Low-Risk Factor allows safe range-of-motion assessment (simple rear-end MVC, sitting position in ED, ambulatory at any time, delayed neck pain onset, or absence of midline C-spine tenderness): if NO low-risk factor exists (= 0), mandate imaging! Step 3 (only reached when High-Risk = 0 AND Low-Risk = 1) asks the patient to actively rotate their neck 45° left and 45° right: if able (= 1), clear the C-spine clinically without radiographs; if unable (= 0), image!
| Step 1: High-Risk Factor? | Step 2: Any Safe Low-Risk Factor? | Step 3: Active 45° Rotation L & R? | Canadian C-Spine Rule Disposition |
|---|---|---|---|
| Yes (1: e.g., Age >= 65) | Not Reached | Do NOT Test ROM! | "Step 1 High Risk: C-Spine Imaging Indicated" |
| No (0) | No (0: Midline tender + high-speed) | Do NOT Test ROM! | "Step 2 No Low-Risk Factor: Imaging Indicated" |
| No (0) | Yes (1: Ambulatory / Sitting in ED) | No (0: Pain stops < 45°) | "Step 3 Failed 45° Rotation: Imaging Indicated" |
| No (0) | Yes (1: Ambulatory / Sitting in ED) | Yes (1: Rotates 45° L & R) | "Clinically Cleared: No C-Spine Radiography Needed" |
3-Step Sequential Canadian C-Spine Rule (CCR) Decision Formula
Evaluates Step 1 High-Risk (B2), Step 2 Low-Risk Presence (B3), and Step 3 Active 45° Rotation (B4) in strict clinical order.
=IF(OR(MIN(B2:B4) < 0, MAX(B2:B4) > 1), "Invalid Binary Flag (0 or 1)", IF(B2 = 1, "Step 1 High Risk: C-Spine Imaging Indicated", IF(B3 = 0, "Step 2 No Low-Risk Factor: Imaging Indicated", IF(B4 = 0, "Step 3 Failed 45 deg Rotation: Imaging Indicated", "Clinically Cleared: No C-Spine Radiography Needed"))))Safe to Assess Active 45° Neck Range of Motion? (Gatekeeper Check)
Confirms High-Risk = 0 AND Low-Risk >= 1 before instructing a trauma patient to rotate their neck.
