BODE Index for COPD 4-Year Survival & Transplant Referral (0–10)

Calculate Bartolome Celli’s 0–10 multidimensional COPD BODE Index from Body-mass index (<= 21), Obstruction (FEV1 % predicted), Dyspnea (mMRC 0–4), and Exercise capacity (6-minute walk distance).

Predict COPD 4-year survival far more accurately than spirometric FEV1 alone!

Chronic obstructive pulmonary disease (COPD) is a systemic wasting and exercise-limiting syndrome—so post-bronchodilator FEV1 % predicted alone misses severe pulmonary cachexia and dynamic hyperinflation! Bartolome R. Celli et al. (New England Journal of Medicine 2004) combined four physiological dimensions into the 0–10 BODE Index: (B) Body-mass index <= 21 kg/m^2 (+1 pt), (O) Airflow Obstruction FEV1 % predicted (>= 65%: 0, 50–64%: 1, 36–49%: 2, <= 35%: 3 pts), (D) mMRC Dyspnea scale (0–1: 0, 2: 1, 3: 2, 4: 3 pts), and (E) Exercise 6-Minute Walk Distance (>= 350 m: 0, 250–349 m: 1, 150–249 m: 2, <= 149 m: 3 pts)!

Four-year survival drops steeply across BODE quartiles: Quartile 1 (Score 0–2) enjoys 80% 4-year survival, Quartile 2 (Score 3–4) 67%, Quartile 3 (Score 5–6) 57%, and Quartile 4 (Score 7–10) plummets to just 18% 4-year survival—making BODE >= 7 a core International Society for Heart and Lung Transplantation (ISHLT) listing criterion!
