Bedside Schwartz Pediatric eGFR Formula (2009 IDMS Creatinine)

Calculate pediatric estimated glomerular filtration rate eGFR = (0.413 * Height_cm) / Serum_Cr_mgdL (mL/min/1.73 m^2) for children aged 1–17 years using IDMS-standardized enzymatic serum creatinine.

Why adult CKD-EPI formulas cannot be used in growing children—and why k dropped to 0.413!

Adult eGFR equations fail in pediatrics because muscle mass—and therefore normal baseline serum creatinine—doubles and triples as a child grows from toddlerhood through adolescence! George J. Schwartz et al. solved this by scaling serum creatinine inversely against standing height (cm), which tracks lean body mass normalized to 1.73 m^2 body surface area.

When clinical laboratories switched from older Jaffe alkaline-picrate assays (which cross-reacted with non-creatinine chromogens and used k = 0.55) to IDMS-traceable enzymatic creatinine assays, Schwartz et al. (Journal of the American Society of Nephrology 2009, CKiD cohort) recalibrated the universal pediatric bedside constant for ages 1–17 years to k = 0.413: eGFR = (0.413 * Height_cm) / Serum_Cr_mgdL! A 125 cm child with enzymatic Cr = 0.65 mg/dL has eGFR = 79.4 mL/min/1.73 m^2 (KDIGO G2), whereas a 110 cm child with Cr = 1.40 mg/dL drops to 32.5 mL/min/1.73 m^2 (KDIGO G3b — requiring immediate vancomycin/aminoglycoside renal dose adjustment)!
