BARD Score (BMI, AST/ALT Ratio, Diabetes) for MASLD Liver Fibrosis
Rule out advanced F3–F4 hepatic bridging fibrosis in NAFLD/MASLD without platelet counts using Stephen Harrison’s 0–4 BARD Score (BMI >= 28 [+1], AST/ALT >= 0.8 [+2], Type 2 Diabetes [+1]; 0–1 has 96% NPV).
Why AST / ALT flipping above 0.8 signals bridging fibrosis in fatty liver disease!
In uncomplicated steatotic liver disease (MASLD / NAFLD), serum ALT comfortably exceeds AST (AST / ALT ~ 0.5–0.7). As sinusoidal hepatic fibrosis advances toward bridging F3–F4 cirrhosis, sinusoidal endothelial clearance of mitochondrial AST drops while hepatocyte ALT synthesis wanes—pushing AST / ALT >= 0.8 even before overt cirrhosis (> 1.0) appears! Stephen A. Harrison et al. (Gut 2008) combined three instantly available clinic variables into the 0–4 BARD Score: (B) BMI >= 28 kg/m^2 (+1 point), (AR) AST / ALT Ratio >= 0.8 (+2 points), and (D) Type 2 Diabetes Mellitus (+1 point)!
Because BARD requires zero specialized collagen biomarkers or platelet counts, primary-care clinics use BARD = 0–1 (96% Negative Predictive Value!) to confidently rule out advanced F3–F4 fibrosis, while BARD = 2–4 (17-fold odds ratio for advanced fibrosis!) triggers FibroScan transient elastography or hepatology referral!
| BARD Total Score (0–4 Points) | F3–F4 Advanced Fibrosis Risk | Primary-Care MASLD Pathway | |
|---|---|---|---|
| BARD 0–1 Points (e.g., BMI 29, AST/ALT 0.61 -> 1) | Low Risk (96% NPV Rule-Out) | Lifestyle Weight Loss & Repeat Score in 1–2 Years | |
| BARD 2–4 Points (e.g., BMI 31.4, AST/ALT 0.92, DM -> 4) | 17x Odds of F3–F4 Fibrosis | Order VCTE (FibroScan) / ELF & Hepatology Referral | |
| Invalid Enzymes (AST <= 0 or ALT <= 0) | Lab Ratio Guard | "Invalid BARD Lab Input" | Requires Positive BMI, AST, and ALT |
Total 3-Component BARD Liver Fibrosis Score (0–4 Points)
Evaluates BMI >= 28 (+1), AST / ALT >= 0.8 (+2), and Diabetes 0/1 (+1) directly from raw vitals and liver enzymes.
=IF(OR(B2 <= 0, B3 <= 0, B4 <= 0, B5 < 0, B5 > 1), "Invalid BARD Lab Input", IF(B2 >= 28, 1, 0) + IF((B3 / B4) >= 0.8, 2, 0) + B5)MASLD Advanced Fibrosis Rule-Out vs. Elastography Referral
Classifies BARD 0–1 as Low Risk (96% NPV) and BARD 2–4 as High Risk requiring FibroScan/hepatology evaluation.
