AUDIT-C Alcohol Misuse Screening Score (0–12) & Sex-Specific Cutoff Formula
Calculate the 3-question WHO/VA AUDIT-C consumption score (0 to 12 points) and apply validated sex-specific hazardous-drinking thresholds (>= 4 points in men, >= 3 points in women).
Screen unhealthy alcohol use in 3 questions with sex-specific >=4 / >=3 thresholds!
Kristen Bush et al. (1998) distilled the 10-item WHO Alcohol Use Disorders Identification Test down to its 3 consumption items—AUDIT-C—which now serves as the standard USPSTF/VA annual primary-care alcohol screen! Each question scores 0 to 4 points (total 0–12): Q1 Drinking Frequency (0 = Never, 1 = Monthly or less, 2 = 2–4x/month, 3 = 2–3x/week, 4 = >= 4x/week), Q2 Typical Drinks on a Drinking Day (0 = 1–2, 1 = 3–4, 2 = 5–6, 3 = 7–9, 4 = >= 10), and Q3 Heavy Episodic ">= 6 Drinks" Frequency (0 = Never, 1 = Less than monthly, 2 = Monthly, 3 = Weekly, 4 = Daily/almost daily)!
Crucial scoring detail: AUDIT-C positive thresholds are SEX-SPECIFIC! A score >= 4 in men or >= 3 in women indicates a positive screen for hazardous drinking or active alcohol use disorder (triggering brief counseling and the remaining 7 AUDIT items), while scores >= 8 in either sex signal high probability of severe alcohol dependence!
| Sex Assigned / Metabolic Cutoff | Negative Screen | Positive Hazardous Screen | High-Severity Tier |
|---|---|---|---|
| Male ("M", Cutoff >= 4) | 0 – 3 points | 4 – 7 points (Brief Intervention) | 8 – 12 points (Dependence Assessment) |
| Female ("F", Cutoff >= 3) | 0 – 2 points | 3 – 7 points (Brief Intervention) | 8 – 12 points (Dependence Assessment) |

Total 3-Item AUDIT-C Consumption Score (0 to 12 Points)
Validates each question lies in [0, 4] (returning 0 automatically if Q1 frequency = 0).

Evaluates sex B2 ("M" or "F") against its validated positive threshold and flags severe scores >= 8.
