ARISCAT Score Formula: Postoperative Pulmonary Complications (PPC) Risk

Calculate Jaume Canet’s 7-factor ARISCAT score (0–123 points) predicting postoperative respiratory failure, pneumonia, and atelectasis across Low (< 26), Intermediate (26–44), and High (>= 45) surgical risk.

Why surgical incision site and anesthesia duration > 3 hours outweigh mild age or anemia in PPC risk!

Jaume Canet et al. (2010, Anesthesiology — ARISCAT prospective cohort of 2,464 surgical patients, externally validated in PERISCOPE across 5,099 European operations) identified 7 independent preoperative and intraoperative predictors of postoperative pulmonary complications (respiratory failure, infection, pleural effusion, bronchospasm, pneumothorax): Age (<= 50: 0; 51–80: 3; > 80: 16), Pre-op room-air SpO2 (>= 96%: 0; 91–95%: 8; <= 90%: 24), Respiratory infection in last month (17), Pre-op anemia Hb <= 10 g/dL (11), Surgical incision (Peripheral: 0; Upper abdominal: 15; Intrathoracic: 24), Surgery duration (< 2h: 0; 2–3h: 16; > 3h: 23), and Emergency procedure (8)!

Total ARISCAT spans 0 to 123 points: Low Risk (< 26 pts, ~1.6% PPC rate), Intermediate Risk (26–44 pts, ~13.3% PPC rate), and High Risk (>= 45 pts, ~42.1% PPC rate—warranting lung-protective ventilation, PEEP/recruitment, neuromuscular reversal monitoring, and post-op CPAP/ICU stepdown)!

Total 7-Factor ARISCAT Postoperative Pulmonary Risk Score (0 to 123)

Sums the 7 weighted ARISCAT domain scores B2:B8, validating item caps.

=IF(OR(MIN(B2:B8) < 0, B2 > 16, B3 > 24, B4 > 17, B5 > 11, B6 > 24, B7 > 23, B8 > 8), "Invalid ARISCAT Component", SUM(B2:B8))

ARISCAT Perioperative PPC Risk Tier (< 26, 26–44, >= 45)

Classifies surgical PPC probability into Low (~1.6%), Intermediate (~13.3%), or High Risk (~42.1%).
