Apfel Simplified PONV Risk Score & Multimodal Prophylaxis Calculator
Calculate the 0–4 Apfel Postoperative Nausea and Vomiting (PONV) score, baseline unprophylaxed PONV probability (10%, 21%, 39%, 61%, 79%), and Fourth Consensus antiemetic prophylaxis tier.
Predict postoperative nausea risk from 4 pre-anesthesia questions and match antiemetic classes!
Postoperative nausea and vomiting (PONV) delays PACU discharge, risks surgical wound dehiscence, and ranks among patients’ most feared anesthesia complications. Christian C. Apfel et al. (Anesthesiology 1999) distilled dozens of perioperative variables into four equally weighted binary (0/1) predictors under volatile general anesthesia: (1) Female sex, (2) Non-smoking status, (3) History of prior PONV or motion sickness, and (4) Planned postoperative opioid analgesia!
Summing 0 to 4 Apfel risk factors maps directly to validated baseline PONV incidences of 10% (0 factors), 21% (1 factor), 39% (2 factors), 61% (3 factors), and 79% (4 factors)! Under the Fourth Consensus PONV Guidelines, each distinct antiemetic drug class (5-HT3 antagonist ondansetron, corticosteroid dexamethasone, NK1 antagonist aprepitant, dopamine antagonist amisulpride, or propofol TIVA) reduces relative risk independently by ~26%—so Apfel = 2 receives dual-agent prophylaxis while Apfel = 3–4 (61–79% risk!) requires >= 3 multimodal interventions plus propofol-based TIVA!
| Apfel Risk Factor Count (0–4) | Baseline Unprophylaxed PONV (%) | Risk Stratification | Consensus Antiemetic Strategy |
|---|---|---|---|
| 0 or 1 Risk Factors | 10% (0) / 21% (1) | Low PONV Risk | Wait-and-See Rescue or Single Agent |
| 2 Risk Factors | 39% | Medium PONV Risk | 2 Distinct Antiemetic Classes (e.g., Dex + Ondansetron) |
| 3 or 4 Risk Factors | 61% (3) / 79% (4) | High PONV Risk | >= 3 Multimodal Classes + Propofol TIVA / Opioid-Sparing |
Predicted Baseline Unprophylaxed PONV Incidence (%)
Maps validated 0–4 Apfel sum across B2:B5 directly to published Apfel (1999) incidence percentages (10, 21, 39, 61, 79).
=IF(OR(COUNT(B2:B5) < 4, MIN(B2:B5) < 0, MAX(B2:B5) > 1), "Invalid Apfel Flag (0/1)", IF(SUM(B2:B5) = 0, 10, IF(SUM(B2:B5) = 1, 21, IF(SUM(B2:B5) = 2, 39, IF(SUM(B2:B5) = 3, 61, 79)))))Fourth Consensus PONV Multimodal Prophylaxis Recommendation
Assigns single/rescue (0–1), dual-class (2), or >= 3 multimodal + TIVA prophylaxis (3–4).
